What are the directions to your office?
We are located on Hollister Drive which intersects with Milwaukee (Route 21) Ave.
about one quarter mile north of Route 60.

If you are coming from the South:
Travel north on Milwaukee Ave, crossing the intersection of Route 60. Continue to travel
north, to the intersection of Milwaukee Ave. and Hollister Drive. Turn Right, heading east,
follow Hollister Drive to our office located at 1800 Hollister Drive, Suite 211.


If you are coming from the North:
Travel south on Milwaukee Ave, crossing the intersection of Route 176. Continue to travel
south, to the intersection of Milwaukee Ave. and Hollister Drive. Turn Left, heading east,
follow Hollister Drive to our office located at 1800 Hollister Drive, Suite 211

Where should I park?
You can park in the lot located directly adjacent to our professional building.

What are your hours?
Our office hours are located in the Location/Hours section of our web site.
If you should need medical attention when our office is closed please contact us at
847-816-3084. If it is a life threatening situation, please call 911.

What are Sick Call Hours?
Sick Call Hours are for acute, minor illnesses such as sore throat,
bronchitis and urinary tract infection.

Patients with more serious medical problems, chronic illnesses or needing
follow-up care are better served by scheduling an appointment so that we may
have sufficient time to evaluate and treat your medical concerns.

No appointment is necessary for sick call hours of 7:15 a.m. to 8:00 a.m.

I am a new patient. What should I bring to my first visit?
You can view and download any forms you will need from our patient forms section of this
web site. All our forms are in pdf format for easy viewing. Prior to your visit please print out
the appropriate forms and fill them out completely. It is very important that you bring the
completed forms along with your current insurance card and picture I. D. with you on your
first appointment visit.

What if I have to cancel my appointment or reschedule?
If you are unable to keep your appointment, kindly give us 24 hours notice at
which time we can reschedule your appointment.

Our receptionists answer all incoming calls between 8:00 a.m. and 4:30 p.m.
Monday through Friday, 8:00 a.m. to 11:30 a.m. on Saturday. They will
schedule, reschedule or cancel your appointment and can answer many of
your questions about our practice.
